ACECLOFENAC + PARACETAMOL + THIOCOLCHICOSIDE
ACTICROSS-TH PLUS TAB combines aceclofenac and paracetamol with thiocolchicoside, a muscle relaxant, for painful musculoskeletal conditions accompanied by spasm. It is used in acute low back pain, cervical and shoulder spasm, torticollis, sprains and strains, and post-traumatic musculoskeletal pain, addressing inflammation, pain and spasm together.

Benefits
- Aceclofenac reduces inflammation and prostaglandin-driven pain at the site of injury.
- Paracetamol adds central analgesia, allowing useful pain control at a lower NSAID burden.
- Thiocolchicoside acts on GABAergic and glycinergic pathways to release muscle spasm — the component an anti-inflammatory alone will not resolve.
- Thiocolchicoside relieves spasm without the marked sedation associated with several older muscle relaxants.
- Treating spasm and inflammation together restores movement and allows physiotherapy to begin sooner.

Side effects
Not everyone gets these. Most are mild and settle as the course finishes.
Common
- Nausea, indigestion or epigastric discomfort
- Diarrhoea or abdominal pain
- Drowsiness or dizziness
- Headache
Less common
- Gastric erosion, ulceration or gastrointestinal bleeding
- Raised liver enzymes
- Skin rash, itching or urticaria
- Vasovagal symptoms with faintness on standing
Stop and get medical help if any of these occur
- Black, tarry stools or vomiting of blood
- Yellowing of the skin or eyes
- Swelling of the face, lips or tongue, or difficulty breathing
- Seizures
- Widespread rash with blistering or peeling skin

Precautions
- Thiocolchicoside is restricted to short courses — no more than seven consecutive days — because of genotoxicity concerns with prolonged exposure.
- Contraindicated in pregnancy, in women of childbearing potential not using reliable contraception, and in breastfeeding.
- Contraindicated in active peptic ulcer, severe hepatic or renal impairment, and NSAID hypersensitivity.
- Contraindicated in patients with a history of seizures, as thiocolchicoside can lower the seizure threshold.
- Check total daily paracetamol intake across all products.
- May cause drowsiness. Warn the patient before driving or operating machinery, and avoid alcohol.
- Use with care in the elderly and alongside anticoagulants, corticosteroids or other NSAIDs.
