ALCITRATE 100ML SYP

DISODIUM HYDROGEN CITRATE 1.37G SYP

ALCITRATE 100ML SYP is an oral syrup of disodium hydrogen citrate 1.37 g, a urinary alkaliniser. It is used to relieve the burning and discomfort of lower urinary tract infection and cystitis, to raise urinary pH in the management of uric acid stones and gout, and to reduce crystal formation in patients prone to recurrent stones.

Benefits

  • Raises urinary pH, which relieves the stinging and burning of an acidic urine in cystitis, often within hours of the first dose.
  • Uric acid is far more soluble in alkaline urine, so alkalinisation helps dissolve existing uric acid stones and prevents new ones forming.
  • Useful adjunct in gout and in patients on urate-lowering treatment, where a persistently acidic urine promotes crystal deposition.
  • Reduces recurrence in patients with a history of uric acid or cystine stones when taken as a maintenance measure.
  • The syrup form is easily diluted in water and encourages the increased fluid intake that stone prevention depends on.

Side effects

Not everyone gets these. Most are mild and settle as the course finishes.

Common
  • Nausea or stomach upset
  • Loose stools or diarrhoea
  • Bloating or belching
  • Unpleasant salty taste
Less common
  • Ankle swelling or fluid retention from the sodium load
  • Rise in blood pressure
  • Muscle cramps or weakness
  • Excessive alkalinisation of the urine, promoting phosphate stone formation
Stop and get medical help if any of these occur
  • Marked swelling of the legs or face, or sudden breathlessness
  • Severe muscle weakness, cramps or an irregular heartbeat
  • Confusion or unusual drowsiness
  • Severe or persistent vomiting
  • Loin pain with fever, or blood in the urine

Precautions

  • Contains a significant sodium load. Contraindicated or to be used only under supervision in hypertension, heart failure, oedema and in patients on a sodium-restricted diet.
  • Contraindicated in severe renal impairment and in urinary tract infection caused by urea-splitting organisms.
  • Use with care alongside potassium-sparing diuretics, ACE inhibitors and angiotensin receptor blockers.
  • Always dilute in water before taking, and take after meals to reduce gastric upset.
  • Alkalinising the urine alters the excretion of several drugs — check for interactions before starting.
  • This treats symptoms and urinary chemistry, not infection; a proven bacterial infection still needs appropriate antibiotic treatment.
  • Maintain a high fluid intake throughout, and use in pregnancy or breastfeeding only on medical advice.
