AMIKACIN 500MG 2ML INJ

AMIRITE 500 INJ is an injection of amikacin 500 mg in 2 ml, an aminoglycoside antibiotic given intramuscularly or by intravenous infusion. It is used in serious Gram-negative infections — septicaemia, complicated urinary tract, intra-abdominal, respiratory, bone and joint, and burn or wound infections — particularly where organisms are resistant to gentamicin.

Benefits

Side effects

Not everyone gets these. Most are mild and settle as the course finishes.

Common
  • Pain, redness or induration at the injection site
  • Nausea or vomiting
  • Headache
  • Rash
Less common
  • Rise in serum creatinine indicating renal impairment
  • Ringing in the ears or a sense of fullness in the ear
  • Dizziness or unsteadiness from vestibular effects
  • Low serum magnesium, potassium or calcium
Stop and get medical help if any of these occur
  • Any new hearing loss, ringing in the ears, or vertigo — these may become permanent
  • A marked fall in urine output
  • Muscle weakness or difficulty breathing
  • Numbness or tingling of the face or extremities
  • Severe rash, facial swelling or difficulty breathing

Precautions

  • Amikacin is nephrotoxic and ototoxic. Renal function must be checked before and during treatment, and hearing assessed where courses are prolonged.
  • Dose must be adjusted to renal function and body weight. Therapeutic drug monitoring is recommended, particularly in the elderly, in renal impairment, and in courses beyond a few days.
  • Contraindicated in known aminoglycoside hypersensitivity and in pre-existing significant hearing impairment or vestibular disease.
  • Contraindicated in pregnancy — aminoglycosides cross the placenta and can cause irreversible congenital deafness.
  • Avoid concurrent loop diuretics, amphotericin B, vancomycin, cisplatin and other nephrotoxic or ototoxic agents wherever possible.
  • Contraindicated or used with extreme caution in myasthenia gravis; aminoglycosides can precipitate neuromuscular blockade.
  • Keep the patient well hydrated throughout treatment, and limit courses to the shortest clinically effective duration.
  • For administration by a qualified healthcare professional only. Do not mix in the same syringe or infusion line as a beta-lactam antibiotic.
