ETORICOXIB 90MG
BCOXIB-90 TAB is a tablet of etoricoxib 90 mg, a selective COX-2 inhibitor. It is used for rheumatoid arthritis, ankylosing spondylitis, and acute gouty arthritis, where the higher dose is needed for the greater inflammatory burden of these conditions.

Benefits
- The 90 mg strength is the dose established for the inflammatory arthritides, where 60 mg is often insufficient.
- Selective COX-2 inhibition gives anti-inflammatory effect with substantially less gastroduodenal ulceration than non-selective NSAIDs.
- Effective in acute gout, where rapid and potent anti-inflammatory action shortens the attack.
- Once-daily dosing simplifies long-term treatment in rheumatoid arthritis and ankylosing spondylitis.
- Does not inhibit platelet function, so it does not add to bleeding risk.

Side effects
Not everyone gets these. Most are mild and settle as the course finishes.
Common
- Dizziness or headache
- Indigestion or epigastric discomfort
- Ankle swelling or fluid retention
- Rise in blood pressure
Less common
- Palpitations
- Raised liver enzymes on testing
- Diarrhoea or nausea
- Skin rash or itching
Stop and get medical help if any of these occur
- Chest pain, breathlessness, or weakness or numbness down one side
- Black, tarry stools or vomiting of blood
- Yellowing of the skin or eyes
- Marked swelling of the legs or sudden weight gain
- Widespread rash with blistering or peeling skin

Precautions
- Contraindicated in established ischaemic heart disease, cerebrovascular disease, peripheral arterial disease and moderate to severe heart failure.
- Contraindicated in uncontrolled hypertension. Cardiovascular risk rises with dose and duration, so the 90 mg strength warrants closer blood pressure monitoring than 60 mg.
- Contraindicated in active peptic ulceration or gastrointestinal bleeding, inflammatory bowel disease, and severe hepatic or renal impairment.
- Contraindicated in NSAID or aspirin hypersensitivity, including aspirin-sensitive asthma.
- Contraindicated in pregnancy and breastfeeding.
- In acute gout, use for the shortest period needed to control the attack rather than continuing at this dose long term.
- Use with care in the elderly, in dehydration, and alongside ACE inhibitors, angiotensin receptor blockers or diuretics.
