DEXTROMETHORPHAN 10MG + CPM 2MG + PHENYLEPHRINE 5MG

BERYCOF-DX 100ML SUSP is an oral suspension combining dextromethorphan 10 mg, chlorpheniramine maleate 2 mg and phenylephrine 5 mg. It is used for the symptomatic relief of dry, irritating cough associated with a cold, together with the accompanying runny nose and nasal congestion. It suppresses cough and does not treat the underlying infection.

Benefits

  • Dextromethorphan acts on the cough centre in the medulla to suppress a dry, non-productive cough that is disturbing sleep.
  • Chlorpheniramine dries the post-nasal drip that often drives night-time coughing, and its sedative effect can help the patient sleep.
  • Phenylephrine constricts the nasal mucosal vessels, relieving blockage and improving breathing.
  • Addresses the cough, the runny nose and the congestion together, replacing several separate preparations.
  • The liquid form allows the dose to be titrated and suits patients who cannot swallow tablets.

Side effects

Not everyone gets these. Most are mild and settle as the course finishes.

Common
  • Drowsiness and sedation
  • Dry mouth, nose and throat
  • Dizziness
  • Nausea
Less common
  • Restlessness or difficulty sleeping from the phenylephrine
  • Palpitations or a rise in blood pressure
  • Blurred vision
  • Difficulty passing urine
  • Constipation
Stop and get medical help if any of these occur
  • Severe drowsiness or difficulty rousing
  • Chest pain, or a markedly irregular or rapid heartbeat
  • Confusion, hallucinations or agitation
  • Difficulty breathing, or wheeze
  • Inability to pass urine

Precautions

  • Only for dry, non-productive cough. A cough that brings up sputum should not be suppressed — clearing it is the point.
  • Contraindicated in uncontrolled hypertension, ischaemic heart disease, significant arrhythmia and hyperthyroidism because of the phenylephrine.
  • Contraindicated in narrow-angle glaucoma, urinary retention and significant prostatic enlargement.
  • Do not give with, or within fourteen days of, a monoamine oxidase inhibitor. Dextromethorphan with an MAOI or serotonergic drug risks serotonin syndrome.
  • Causes drowsiness. Warn the patient before driving, and avoid alcohol and other sedatives.
  • Not for children below the age stated on the label without paediatric advice, and use in pregnancy or breastfeeding only on medical advice.
  • Reassess if cough persists beyond a week, or is accompanied by fever, breathlessness or blood-stained sputum.
  • Dextromethorphan is subject to misuse at high doses — dispense and use only as prescribed.
