BESCORIL-AM 100SYP SF is a sugar-free oral syrup combining ambroxol 15 mg, terbutaline 1.25 mg, guaiphenesin 50 mg and menthol. It is used for productive cough with thick sputum and associated bronchospasm, and the sugar-free base suits patients who need to limit sugar intake, including those with diabetes.

Benefits

  • Ambroxol is a mucolytic that thins sputum and stimulates surfactant production, easing sputum clearance from the smaller airways.
  • Guaiphenesin increases secretion volume and lowers viscosity, making cough more productive.
  • Terbutaline relaxes bronchial smooth muscle, relieving the wheeze and tightness that accompany a chest infection.
  • Menthol soothes throat irritation and improves palatability.
  • The sugar-free base suits patients with diabetes and those advised to restrict added sugar, without changing the therapeutic effect.

Side effects

Not everyone gets these. Most are mild and settle as the course finishes.

Common
  • Fine tremor, particularly of the hands
  • Palpitations or a raised heart rate
  • Nausea or stomach upset
  • Headache or restlessness
Less common
  • Muscle cramps
  • Low serum potassium with higher or repeated doses
  • Altered taste or dry mouth
  • Skin rash or itching
Stop and get medical help if any of these occur
  • Chest pain, or a fast or markedly irregular heartbeat
  • Worsening breathlessness or wheeze after a dose
  • Severe muscle weakness or cramps
  • Swelling of the face, lips or tongue, or difficulty breathing
  • Widespread rash with blistering or peeling skin

Precautions

  • Sugar-free does not mean suitable without supervision in diabetes — the patient still needs their usual glycaemic monitoring.
  • Tremor and palpitations from the terbutaline are expected and usually settle.
  • Use with caution in ischaemic heart disease, arrhythmia, hypertension, hyperthyroidism and diabetes.
  • Beta-agonists can lower serum potassium, particularly alongside diuretics, corticosteroids or xanthines.
  • Contraindicated in known hypersensitivity to any component; use with care in active peptic ulcer disease.
  • Not for a dry, non-productive cough, and not to be combined with a cough suppressant.
  • Maintain a good fluid intake throughout.
  • Use in pregnancy and breastfeeding only on medical advice.
