AMBROXYL HCL 30MG + GUAIPHENESIN 50MG + LEVOSALBUTAMOL 1MG

BESCORIL-LS 100ML SYRUP is an oral syrup combining ambroxol hydrochloride 30 mg, guaiphenesin 50 mg and levosalbutamol 1 mg. It is used for productive cough with thick sputum accompanied by bronchospasm, in acute and chronic bronchitis and in cough associated with asthma or chronic obstructive pulmonary disease.

Benefits

  • Levosalbutamol is the active isomer of salbutamol, giving bronchodilation with less tremor and tachycardia than the racemic mixture at equivalent effect.
  • Ambroxol thins tenacious sputum and stimulates surfactant, easing clearance from the smaller airways.
  • Guaiphenesin increases secretion volume and reduces viscosity, making the cough more productive.
  • Opening the airways while thinning secretions makes each cough more effective.
  • The higher 30 mg ambroxol strength suits adults with more tenacious sputum.

Side effects

Not everyone gets these. Most are mild and settle as the course finishes.

Common
  • Tremor, generally milder than with racemic salbutamol
  • Palpitations or a raised heart rate
  • Nausea or stomach upset
  • Headache
Less common
  • Muscle cramps
  • Low serum potassium with higher or repeated doses
  • Restlessness or difficulty sleeping
  • Dry mouth or altered taste
Stop and get medical help if any of these occur
  • Chest pain, or a fast or markedly irregular heartbeat
  • Worsening breathlessness or wheeze after a dose
  • Severe muscle weakness or cramps
  • Swelling of the face, lips or tongue, or difficulty breathing
  • Coughing up blood

Precautions

  • Use with caution in ischaemic heart disease, arrhythmia, hypertension, hyperthyroidism and diabetes.
  • Beta-agonists lower serum potassium, particularly alongside diuretics, corticosteroids or xanthines — monitor in patients on those drugs.
  • Contraindicated in known hypersensitivity to any component; use with care in active peptic ulcer disease.
  • Not for dry, non-productive cough, and not to be combined with a cough suppressant.
  • Maintain a good fluid intake, which is what makes the mucolytic effective.
  • This is a symptomatic treatment. Worsening asthma or COPD needs its underlying therapy reviewed, not just this syrup.
  • Reassess if cough persists beyond a week or is accompanied by fever, breathlessness or blood-stained sputum.
  • Use in pregnancy and breastfeeding only on medical advice.
