LEVOSALBUTAMOL 1MG + AMBROXYL HCL 30MG + GUAIPHENESIN 50MG

BESCORIL-LS JUNIOR SYP is a paediatric syrup combining levosalbutamol 1 mg, ambroxol hydrochloride 30 mg and guaiphenesin 50 mg. It is used for productive cough with thick sputum and associated wheeze in children, with the dose set by age and body weight by the prescriber.

Benefits

  • Levosalbutamol is the active isomer of salbutamol and produces bronchodilation with less tremor and tachycardia, which is useful in children.
  • Ambroxol thins tenacious sputum and stimulates surfactant, aiding clearance from the small airways.
  • Guaiphenesin makes the cough more productive by increasing secretion volume and lowering viscosity.
  • One preparation covers bronchodilation, mucolysis and expectoration rather than three.
  • The syrup allows dosing by weight and is generally well tolerated.

Side effects

Not everyone gets these. Most are mild and settle as the course finishes.

Common
  • Mild tremor
  • Fast heartbeat or palpitations
  • Restlessness or difficulty settling
  • Nausea or stomach upset
Less common
  • Muscle cramps
  • Low serum potassium with repeated dosing
  • Headache
  • Skin rash
Stop and get medical help if any of these occur
  • Fast, pounding or irregular heartbeat
  • Worsening breathlessness or wheeze after a dose
  • Severe agitation or unusual drowsiness
  • Fast breathing with chest indrawing, or difficulty feeding
  • Swelling of the face or lips, or difficulty breathing

Precautions

  • Dose strictly by age and weight using the measure supplied; never substitute an adult preparation.
  • Not for children below the age stated on the label without paediatric advice.
  • Use with caution in children with cardiac disease, hyperthyroidism or diabetes.
  • Do not combine with a cough suppressant, or with another preparation containing the same ingredients.
  • Maintain good fluid intake throughout the course.
  • A child with worsening or recurrent wheeze needs assessment for asthma rather than repeated courses of this syrup.
  • Seek review urgently for fast breathing, chest indrawing, difficulty feeding or persistent fever.
