AMOXY 500 + CLAUV POTAS 125MG + LACTOBACILLUS

BESTOMOX CV 625 LB TAB combines amoxicillin 500 mg and clavulanate potassium 125 mg with lactobacillus, a probiotic included to offset the gut disturbance the antibiotic causes. It covers the same infections as amoxicillin-clavulanate alone and suits patients who have previously had troublesome antibiotic-associated diarrhoea.

Benefits

  • Clavulanate protects amoxicillin from beta-lactamase, extending cover to resistant organisms.
  • Lactobacillus helps re-establish the gut flora that broad-spectrum antibiotics deplete.
  • Reduces the incidence and severity of antibiotic-associated diarrhoea, which is the commonest reason this combination is abandoned mid-course.
  • Particularly useful in the elderly and in patients with a history of gastrointestinal upset on antibiotics.
  • Covers the mixed flora of dental, bite-wound and skin infection alongside common respiratory pathogens.

Side effects

Not everyone gets these. Most are mild and settle as the course finishes.

Common
  • Diarrhoea or loose stools
  • Nausea or vomiting
  • Bloating or flatulence
  • Abdominal discomfort
Less common
  • Oral or vaginal thrush after the course
  • Raised liver enzymes on testing
  • Skin rash
  • Constipation
Stop and get medical help if any of these occur
  • Swelling of the face, lips or tongue, difficulty breathing, or collapse
  • Yellowing of the skin or eyes, dark urine, or pale stools
  • Severe, watery or bloody diarrhoea during or in the weeks after the course
  • Widespread rash with blistering or peeling skin
  • Unusual bruising or bleeding

Precautions

  • Contraindicated in known penicillin allergy, and in any previous amoxicillin-clavulanate-associated jaundice or hepatic dysfunction.
  • The probiotic component should be avoided in severely immunocompromised patients, in central venous catheter carriers, and in the critically ill.
  • Clavulanate carries a risk of cholestatic jaundice, more so in the elderly and with prolonged courses.
  • Take with food to improve clavulanate absorption and reduce diarrhoea.
  • Dose reduction is required in significant renal impairment.
  • Avoid in suspected infectious mononucleosis.
  • Complete the full prescribed course even once symptoms settle.
  • Use with caution where there is a cephalosporin allergy.
