CETROSS-COLD 555 B/L is a tablet combining caffeine 30 mg, diphenhydramine 25 mg, paracetamol 500 mg and phenylephrine 5 mg. It is a night-time cold preparation for fever, aches, runny nose and nasal blockage, in which the sedating antihistamine is intended to help the patient sleep through the illness.

Benefits

  • Paracetamol 500 mg gives a full adult dose for the fever, headache and body ache of a cold.
  • Diphenhydramine dries the runny nose and its sedative effect helps the patient sleep when symptoms are disturbing rest.
  • Phenylephrine relieves nasal blockage, which is often what prevents sleep.
  • Caffeine offsets some of the daytime heaviness of the antihistamine and modestly enhances the analgesic effect.
  • A single tablet covers the full symptom complex over a short course.

Side effects

Not everyone gets these. Most are mild and settle as the course finishes.

Common
  • Marked drowsiness from the diphenhydramine
  • Dry mouth, nose and throat
  • Dizziness
  • Restlessness or difficulty sleeping from the caffeine and phenylephrine
Less common
  • Blurred vision
  • Difficulty passing urine
  • Constipation
  • Palpitations or a rise in blood pressure
Stop and get medical help if any of these occur
  • Chest pain, or a markedly irregular or rapid heartbeat
  • Severe drowsiness or confusion
  • Yellowing of the skin or eyes
  • Swelling of the face, lips or tongue, or difficulty breathing
  • Inability to pass urine

Precautions

  • Diphenhydramine causes marked sedation. The patient must not drive or operate machinery, and the combination with caffeine does not make it safe to do so.
  • Contraindicated in narrow-angle glaucoma, urinary retention, significant prostatic enlargement and pyloroduodenal obstruction.
  • Contraindicated in uncontrolled hypertension, ischaemic heart disease, significant arrhythmia and hyperthyroidism.
  • Do not give with, or within fourteen days of, a monoamine oxidase inhibitor.
  • Check total daily paracetamol intake across all products.
  • Avoid alcohol and other sedatives entirely — the effects are strongly additive.
  • Use with particular caution in the elderly, who are more susceptible to confusion, urinary retention and falls with diphenhydramine.
  • For short-term use only, and not in children without paediatric advice.
