CLOBETASOLE GM + MICONAZOLE + NEOMYCIN
CLOBETOL-GM CREAM 10GM (W/G) is a topical combination cream containing clobetasol, miconazole and neomycin. It is used for inflammatory skin conditions complicated by fungal or bacterial infection — infected eczema, infected intertrigo and mixed dermatoses — where inflammation and infection are present together.

Benefits
- Clobetasol, a potent corticosteroid, rapidly suppresses itching, redness and inflammation.
- Miconazole covers dermatophytes and Candida, the fungal organisms of skin-fold and moist-area infection.
- Neomycin adds cover against common Gram-positive and Gram-negative skin bacteria in secondarily infected lesions.
- Treating inflammation and infection together avoids layering separate preparations on already inflamed skin.
- Useful in infected eczema and intertrigo, where a steroid alone would worsen the infection.

Side effects
Not everyone gets these. Most are mild and settle as the course finishes.
Common
- Burning or stinging on application
- Local redness or itching
- Dryness or peeling of the treated skin
- Greasy residue
Less common
- Skin thinning, visible small blood vessels and stretch marks from the potent steroid
- Contact allergy to neomycin, which is common and presents as worsening rather than improving dermatitis
- Increased hair growth or acneiform eruption in the treated area
- Worsening of an untreated fungal infection
Stop and get medical help if any of these occur
- Rash that spreads or worsens after starting treatment
- Severe blistering, crusting or ulceration
- Marked skin thinning, or the appearance of stretch marks
- Any change in hearing — neomycin absorbed from large or broken areas can be ototoxic
- Swelling of the face, lips or tongue, or difficulty breathing

Precautions
- Contains a potent corticosteroid. Use for short defined courses only — typically no more than two weeks — and never as an ongoing skin cream.
- Do not use on the face, genitals or in occluded skin folds for prolonged periods; steroid atrophy occurs fastest there.
- Neomycin is a common contact allergen. If the rash worsens rather than improves, suspect neomycin sensitisation and stop.
- Do not apply to large areas, to broken or ulcerated skin, or under occlusion — absorbed neomycin can cause ototoxicity and nephrotoxicity.
- For external use only. Avoid the eyes and mucous membranes.
- Contraindicated in known hypersensitivity to any component, in viral skin infection such as herpes, and in tuberculous or syphilitic skin lesions.
- A potent steroid on an undiagnosed fungal rash masks inflammation while the infection spreads.
- Use in pregnancy, breastfeeding and in children only on medical advice.
