CLOBETASOLE + NEOMYCIN + TOLNAFTATE + IODOCHLORHYD + KETOCONAZOLE
DERMIBEST-5 CREAM 15GM is a topical five-component cream containing clobetasol, neomycin, tolnaftate, iodochlorhydroxyquinoline and ketoconazole. It is used for mixed or infected inflammatory skin conditions where fungal and bacterial organisms may both be present alongside marked inflammation.

Benefits
- Ketoconazole and tolnaftate together cover a broad range of dermatophytes and yeasts.
- Neomycin and iodochlorhydroxyquinoline provide antibacterial cover in secondarily infected lesions.
- Clobetasol, a potent corticosteroid, rapidly suppresses the itching and inflammation that make these conditions distressing.
- Iodochlorhydroxyquinoline has both antibacterial and antifungal activity and a mild astringent effect useful in weeping lesions.
- A single preparation for a mixed infection avoids layering several topicals on inflamed skin.

Side effects
Not everyone gets these. Most are mild and settle as the course finishes.
Common
- Burning or stinging on application
- Local redness or itching
- Dryness or peeling of the treated skin
- Yellow staining of skin and clothing from iodochlorhydroxyquinoline
Less common
- Skin thinning, visible small blood vessels and stretch marks from the potent steroid
- Contact allergy to neomycin, presenting as worsening dermatitis
- Increased hair growth or acneiform eruption
- Worsening of an untreated fungal infection
Stop and get medical help if any of these occur
- Rash that spreads or worsens after starting treatment
- Severe blistering, crusting or ulceration
- Marked skin thinning, or the appearance of stretch marks
- Any change in hearing
- Swelling of the face, lips or tongue, or difficulty breathing

Precautions
- Contains a potent corticosteroid. Short defined courses only — typically no more than two weeks — never as an ongoing skin cream.
- Do not use on the face, genitals or in occluded skin folds for prolonged periods.
- Do not apply to large areas or broken skin — absorbed neomycin can be ototoxic and nephrotoxic, and iodochlorhydroxyquinoline can interfere with thyroid function tests.
- Neomycin is a common contact allergen; worsening rather than improvement suggests sensitisation.
- For external use only. Avoid the eyes and mucous membranes. Stains clothing and bedding yellow.
- Contraindicated in known hypersensitivity to any component, in viral skin infection, and in tuberculous or syphilitic skin lesions.
- A five-component preparation is rarely the right first choice — a specific diagnosis and a targeted single agent is preferable where possible.
- Use in pregnancy, breastfeeding and in children only on medical advice.
