DOXYCYCLINE 100MG + LACTIC ACID BACILLUS
DOXYROS-SL CAPS B/L is a capsule of doxycycline 100 mg with lactic acid bacillus, a tetracycline antibiotic with a probiotic to offset gut disturbance. It is used for respiratory infections, acne, rickettsial and other zoonotic infections, sexually transmitted infections, and as malaria prophylaxis.

Benefits
- Broad-spectrum activity covering atypical organisms, rickettsiae, Chlamydia, Mycoplasma and spirochaetes that many other antibiotics miss.
- First-line treatment for scrub typhus and other rickettsial illnesses, which are common causes of undifferentiated fever in India.
- Established treatment for moderate inflammatory acne, where its anti-inflammatory effect matters as much as its antibacterial action.
- Lactic acid bacillus helps re-establish the gut flora the antibiotic depletes, reducing diarrhoea.
- Once- or twice-daily dosing, with absorption less affected by food than older tetracyclines.

Side effects
Not everyone gets these. Most are mild and settle as the course finishes.
Common
- Nausea or vomiting
- Diarrhoea or loose stools
- Increased sensitivity to sunlight, with easy sunburn
- Abdominal discomfort
Less common
- Oesophageal irritation or ulceration if taken without enough water or lying down
- Oral or vaginal thrush after the course
- Headache
- Skin rash
Stop and get medical help if any of these occur
- Severe pain on swallowing or behind the breastbone
- Severe, watery or bloody diarrhoea during or in the weeks after the course
- Severe sunburn after limited exposure
- Severe headache with visual disturbance, which may indicate raised intracranial pressure
- Widespread rash with blistering or peeling skin

Precautions
- Contraindicated in children under eight years and in pregnancy — tetracyclines cause permanent tooth discolouration and affect bone growth.
- Contraindicated in breastfeeding and in known tetracycline hypersensitivity.
- Take with a full glass of water and remain upright for at least thirty minutes afterwards. Taking it lying down or with too little water causes oesophageal ulceration.
- Causes marked photosensitivity. Counsel on sun avoidance and sunscreen; this is a frequent and avoidable problem.
- Separate from antacids, calcium, iron, zinc and dairy products by at least two hours, as these bind doxycycline and prevent absorption.
- The probiotic component should be avoided in severely immunocompromised patients and in the critically ill.
- May reduce the reliability of oral contraceptives if vomiting or diarrhoea occurs.
- Complete the full prescribed course, and use with caution alongside warfarin, whose effect it can potentiate.
