LINEZOLID 600MG

LIZOLINE 600 TAB S/P MC is a tablet of linezolid 600 mg, an oxazolidinone antibiotic. It is a reserve agent used for serious infections caused by resistant Gram-positive organisms — MRSA, vancomycin-resistant enterococci, and multidrug-resistant tuberculosis — including pneumonia and complicated skin and soft-tissue infection.

Benefits

  • Active against MRSA and vancomycin-resistant enterococci, organisms that resist most other oral agents.
  • Near-complete oral bioavailability, so the tablet achieves blood levels equivalent to intravenous dosing — allowing early discharge on oral therapy.
  • Excellent penetration into lung tissue and skin, the sites where resistant Gram-positive infection most often needs treating.
  • Part of the regimen for multidrug-resistant and extensively drug-resistant tuberculosis.
  • Requires no dose adjustment in renal impairment.

Side effects

Not everyone gets these. Most are mild and settle as the course finishes.

Common
  • Nausea or vomiting
  • Diarrhoea
  • Headache
  • Altered or metallic taste
Less common
  • Low platelet, white cell or red cell counts
  • Oral or vaginal thrush
  • Dizziness
  • Tongue discolouration
Stop and get medical help if any of these occur
  • Unusual bruising or bleeding, or bleeding gums
  • New numbness, tingling or weakness in the hands or feet
  • Blurred vision, loss of colour vision or any change in visual acuity
  • Nausea, vomiting and rapid breathing, which may indicate lactic acidosis
  • Agitation, confusion, fever, sweating, tremor or muscle rigidity — this may be serotonin syndrome
  • Severe, watery or bloody diarrhoea

Precautions

  • Linezolid is a reserve antibiotic. Use it only where a resistant Gram-positive organism is confirmed or strongly suspected — indiscriminate use will destroy one of the few remaining oral options for MRSA.
  • Causes reversible bone marrow suppression, particularly beyond two weeks of treatment. Check full blood counts weekly during treatment.
  • Peripheral and optic neuropathy occur with prolonged use and may be irreversible. Any visual change or new numbness needs immediate review.
  • Linezolid is a weak monoamine oxidase inhibitor. Risk of serotonin syndrome with SSRIs, SNRIs, tricyclics, triptans and tramadol — avoid or monitor closely.
  • Avoid tyramine-rich foods — aged cheese, fermented and cured products, soy sauce, draught beer — which can precipitate a hypertensive reaction.
  • Do not give with, or within two weeks of, a monoamine oxidase inhibitor. Avoid sympathomimetics including pseudoephedrine and phenylephrine.
  • Courses beyond 28 days should be exceptional and closely supervised.
  • Use in pregnancy and breastfeeding only where clearly indicated.
