MECAROSE PLUS CAP NEW is a capsule combining methylcobalamin 1500 mcg, alpha lipoic acid 100 mg, folic acid 1.5 mg and pyridoxine 3 mg. It is used in peripheral neuropathy — particularly diabetic neuropathy — and in vitamin B12 deficiency with neurological features.

Benefits
- Methylcobalamin is the active coenzyme form of vitamin B12, directly involved in myelin formation and nerve repair.
- Alpha lipoic acid is an antioxidant with evidence of benefit in reducing the burning and tingling of diabetic neuropathy.
- Pyridoxine and folic acid support nerve metabolism and lower homocysteine, which is implicated in nerve and vascular damage.
- Addresses the symptomatic burden of neuropathy — burning, tingling and numbness — where conventional analgesics are ineffective.
- Useful in the deficiency states common in vegetarian diets, in the elderly and in long-term metformin users.

Side effects
Not everyone gets these. Most are mild and settle as the course finishes.
Common
- Nausea or mild stomach upset
- Altered taste
- Loose stools
- Headache
Less common
- Skin rash or itching
- Abdominal cramping
- Reduced blood glucose in diabetic patients, from the alpha lipoic acid
- Dizziness
Stop and get medical help if any of these occur
- Symptoms of hypoglycaemia — sweating, tremor, confusion — in a patient on diabetes medication
- Swelling of the face, lips or tongue, or difficulty breathing
- Widespread rash or hives
- Worsening numbness or weakness despite treatment
- New numbness or tingling on prolonged high-dose pyridoxine

Precautions
- Alpha lipoic acid can lower blood glucose. Patients on insulin or sulphonylureas need monitoring and may need a dose adjustment.
- Folic acid can correct the anaemia of B12 deficiency while allowing neurological damage to progress. This combination contains B12, but the principle matters — confirm B12 status before relying on folate.
- Prolonged high-dose pyridoxine causes peripheral neuropathy — the very condition being treated. Do not exceed the recommended dose or continue indefinitely without review.
- Neuropathy needs a diagnosis. Diabetes, B12 deficiency, alcohol, thyroid disease and nerve compression all require different management.
- Take with food to reduce nausea.
- In diabetic neuropathy this supports but does not replace glycaemic control, which remains the main determinant of progression.
- Contraindicated in known hypersensitivity to any component; use in pregnancy and breastfeeding only on medical advice.
