MECAROSE PLUS2ML INJ +SYRNG is a 2 ml prefilled injection combining pyridoxine hydrochloride, methylcobalamin and nicotinamide, given intramuscularly. It is used in peripheral neuropathy, vitamin B12 deficiency with neurological features, and in B-vitamin deficiency where oral absorption is impaired.

Benefits
- The injectable route bypasses the gut, essential in pernicious anaemia and malabsorption where oral B12 is not absorbed.
- Methylcobalamin is the active coenzyme form of vitamin B12, involved directly in myelin formation and nerve repair.

Side effects
Not everyone gets these. Most are mild and settle as the course finishes.
Common
- Pain or redness at the injection site
- Flushing or warmth of the skin from the nicotinamide
- Nausea
- Headache
Less common
- Itching or rash
- Dizziness
- Transient fall in blood pressure
- Metallic taste
Stop and get medical help if any of these occur
- Swelling of the face, lips or tongue, difficulty breathing, or collapse
- Widespread rash or hives
- Severe flushing with dizziness or fainting
- New numbness or tingling on prolonged high-dose pyridoxine
- Severe pain, swelling or redness at the injection site

Precautions
- For administration by a qualified healthcare professional, by intramuscular injection.
- Establish the cause of B12 deficiency before treating; pernicious anaemia requires lifelong replacement.
- Correcting B12 deficiency can unmask hypokalaemia as red cell production resumes — monitor in severe deficiency.
- Prolonged high-dose pyridoxine causes peripheral neuropathy; do not continue courses indefinitely without review.
- Nicotinamide flushing is common and harmless but alarming — warn the patient.
- Contraindicated in known hypersensitivity to any component or to benzyl alcohol, present as a preservative.
- Contraindicated in neonates because of the benzyl alcohol content.
- Use in pregnancy and breastfeeding only on medical advice.
