MEFENAMIC ACID + DICYCLOMINE

MEFROS SPASS TAB B/L is a tablet combining mefenamic acid with dicyclomine, an NSAID paired with a smooth-muscle antispasmodic. It is used for colicky and spasmodic pain — dysmenorrhoea, intestinal colic and spasmodic abdominal pain — where relaxing the smooth muscle matters as much as blocking the pain.

Benefits

  • Mefenamic acid is particularly effective in dysmenorrhoea, reducing both the pain and the excessive uterine contraction that causes it.
  • Dicyclomine relaxes gastrointestinal and uterine smooth muscle directly, relieving the cramping component.
  • The two mechanisms are complementary in period pain, which is the commonest indication.
  • Mefenamic acid also reduces menstrual blood loss, an advantage in heavy periods.
  • Useful in intestinal colic and in the spasmodic pain of irritable bowel syndrome.

Side effects

Not everyone gets these. Most are mild and settle as the course finishes.

Common
  • Dry mouth
  • Diarrhoea, which is more frequent with mefenamic acid
  • Blurred vision
  • Dizziness or drowsiness
Less common
  • Nausea or epigastric discomfort
  • Difficulty passing urine
  • Constipation
  • Fast heart rate or palpitations
Stop and get medical help if any of these occur
  • Severe or persistent diarrhoea
  • Black, tarry stools or vomiting of blood
  • Inability to pass urine
  • Severe eye pain with blurred vision and halos around lights
  • Confusion or hallucinations, particularly in the elderly
  • Severe abdominal pain that worsens rather than settles

Precautions

  • Contraindicated in narrow-angle glaucoma, urinary retention, significant prostatic enlargement, myasthenia gravis, paralytic ileus and obstructive gastrointestinal disease.
  • Contraindicated in active peptic ulcer disease, inflammatory bowel disease, severe hepatic or renal impairment, and NSAID hypersensitivity.
  • Diarrhoea with mefenamic acid is a reason to stop rather than continue.
  • Do not use to mask undiagnosed acute abdominal pain, which may need surgical assessment.
  • Contraindicated in infants under six months and in breastfeeding, because of the dicyclomine.
  • Causes drowsiness and blurred vision. Warn before driving.
  • Use with particular caution in the elderly, who are more susceptible to confusion, urinary retention and falls.
  • Avoid in the third trimester of pregnancy; limit to short courses and take with or after food.
