PANTANXT 40 TAB
PANTOPRAZOLE 40MG
PANTANXT 40 TAB is a vial of pantoprazole 40 mg for intravenous injection or infusion, a proton pump inhibitor. It is used in hospital for acute upper gastrointestinal bleeding, stress ulcer prophylaxis, severe reflux oesophagitis, and where the oral route is unavailable.

Benefits
- Rapid and profound acid suppression by the intravenous route, where oral dosing is impossible or too slow.
- High-dose intravenous proton pump inhibitor therapy is standard after endoscopic treatment of a bleeding peptic ulcer, reducing rebleeding.
- Raising gastric pH stabilises clot formation at a bleeding ulcer site.
- Used for stress ulcer prophylaxis in critically ill patients at risk of gastrointestinal bleeding.
- Pantoprazole has fewer drug interactions than omeprazole, an advantage in complex inpatients.

Side effects
Not everyone gets these. Most are mild and settle as the course finishes.
Common
- Headache
- Diarrhoea
- Nausea
- Pain or thrombophlebitis at the infusion site
Less common
- Abdominal pain or flatulence
- Dizziness
- Raised liver enzymes
- Skin rash
Stop and get medical help if any of these occur
- Severe or persistent diarrhoea, which may indicate Clostridioides difficile infection
- Severe muscle cramps, weakness, tremor or an irregular heartbeat, which may indicate low magnesium
- Continued vomiting of blood or passage of black stools
- Widespread rash with blistering or peeling skin
- Reduced urine output or blood in the urine

Precautions
- For administration by a qualified healthcare professional. Reconstitute and dilute as directed, and give by slow injection over at least two minutes or by infusion.
- Acid suppression treats the acid, not the bleeding — active upper gastrointestinal haemorrhage requires endoscopic assessment and management.
- Switch to the oral route as soon as the patient can take it; intravenous therapy is not intended to continue longer than necessary.
- Prolonged use is associated with low magnesium, low vitamin B12 and enteric infection.
- Alarm features and any suspicion of gastric malignancy need investigation, as acid suppression can mask it.
- Dose reduction may be required in significant hepatic impairment.
- Check compatibility before mixing with any other infusion fluid or additive.
- Use in pregnancy and breastfeeding only where clearly indicated.
