PARACETAMOL 125MG
PYRIBEST SUSP 60ML is an oral suspension of paracetamol 125 mg per dose, a paediatric analgesic and antipyretic. It is used for fever and mild to moderate pain in children, with the dose set by body weight, and is the most widely used medicine in childhood.

Benefits
- The liquid form allows dosing by body weight, which is essential in children.
- Effective for fever, teething pain, post-immunisation discomfort, headache and minor injury.
- Does not irritate the stomach or affect platelet function, unlike NSAIDs.
- Suitable where ibuprofen is contraindicated, including in asthma, dehydration and chickenpox.
- The best-established safety record of any medicine used in childhood when dosed correctly.

Side effects
Not everyone gets these. Most are mild and settle as the course finishes.
Common
- Generally very well tolerated at recommended doses
- Occasional nausea
- Mild stomach discomfort
- Skin rash in a small number of children
Less common
- Raised liver enzymes on testing
- Itching or urticaria
- Vomiting
- Hypersensitivity reaction
Stop and get medical help if any of these occur
- Any suspected overdose — even when the child appears completely well
- Yellowing of the skin or eyes, or dark urine
- Swelling of the face, lips or tongue, or difficulty breathing
- Widespread rash with blistering or peeling skin
- Fever with drowsiness, a stiff neck, a rash that does not fade under pressure, or difficulty breathing

Precautions
- Dose strictly by body weight, not by age alone, using the measure supplied. Never use a household spoon.
- Do not give alongside any other paracetamol-containing preparation, including cold and cough syrups. Duplication is the commonest cause of paediatric paracetamol overdose.
- Never exceed the stated number of doses in 24 hours, and observe the minimum interval between doses.
- In suspected overdose, seek emergency care immediately regardless of how well the child appears — the antidote works only if given early.
- Reduce the dose in low body weight, malnutrition and hepatic impairment.
- Fever alone is not an indication to treat — treat the child's discomfort, not the number on the thermometer.
- A child with fever plus drowsiness, a non-blanching rash, stiff neck, fast breathing or poor feeding needs urgent assessment, not just an antipyretic.
- Shake well before each dose and keep out of reach of children.
