RABEPRAZOLE 20MG + LEVOSULPRIDE 75MG

RABERITE-L CAP is a capsule combining rabeprazole 20 mg with levosulpiride 75 mg, a proton pump inhibitor paired with a prokinetic and antiemetic. It is used for gastro-oesophageal reflux and functional dyspepsia with prominent nausea, bloating and early fullness.

Benefits

  • Rabeprazole produces rapid and sustained acid suppression, healing oesophagitis and relieving reflux.
  • Levosulpiride is a dopamine D2 antagonist that accelerates gastric emptying and has a central antiemetic effect.
  • Particularly useful in functional dyspepsia and diabetic gastroparesis, where delayed emptying drives the symptoms.
  • The antiemetic action helps where nausea is a prominent complaint.
  • One capsule replaces two separate medicines.

Side effects

Not everyone gets these. Most are mild and settle as the course finishes.

Common
  • Drowsiness or sedation
  • Headache
  • Dry mouth
  • Breast tenderness or enlargement, and milk secretion, from raised prolactin
Less common
  • Menstrual irregularity or absence of periods
  • Reduced libido
  • Diarrhoea or constipation
  • Weight gain
  • Restlessness
Stop and get medical help if any of these occur
  • Muscle stiffness, tremor, restlessness, or involuntary movements of the face and tongue
  • Any persistent involuntary movement — tardive dyskinesia may be irreversible
  • High fever with muscle rigidity, confusion and sweating
  • Severe muscle cramps, weakness or confusion, which may indicate low magnesium
  • Black or tarry stools, or vomiting of blood

Precautions

  • Levosulpiride is a dopamine antagonist and can cause extrapyramidal effects — parkinsonism, acute dystonia and, with prolonged use, tardive dyskinesia, which may be permanent. Restrict to short courses and review regularly.
  • Risk of extrapyramidal effects is much higher in the elderly and with prolonged use. Avoid long-term treatment.
  • Contraindicated in Parkinson's disease, in phaeochromocytoma, in prolactin-dependent tumours and in epilepsy.
  • Contraindicated in gastrointestinal haemorrhage, mechanical obstruction or perforation.
  • Raised prolactin commonly causes breast symptoms, milk secretion and menstrual disturbance — warn the patient before starting.
  • Causes drowsiness. Warn before driving, and avoid alcohol.
  • Take thirty to sixty minutes before food, and swallow whole.
  • Alarm features need investigation before starting acid suppression.
