B12 INJ CYANOCOBALAMIN 500MG INJ
VITAMAX INJ 30ML is a 30 ml multi-dose vial of cyanocobalamin injection, vitamin B12. It is used for the treatment of vitamin B12 deficiency, particularly pernicious anaemia and malabsorption states where oral supplementation cannot be absorbed.

Benefits
- The injectable route bypasses the gut entirely — essential in pernicious anaemia, where the absence of intrinsic factor makes oral B12 useless.
- Corrects megaloblastic anaemia reliably, with reticulocytosis usually apparent within a week.
- Treats the neurological features of B12 deficiency — numbness, tingling, unsteadiness and cognitive change — which can otherwise become permanent.
- Also indicated after gastrectomy, in ileal resection or disease, and in long-term metformin or proton pump inhibitor use.
- The multi-dose vial suits clinics giving regular maintenance injections.

Side effects
Not everyone gets these. Most are mild and settle as the course finishes.
Common
- Pain or redness at the injection site
- Nausea
- Headache
- Dizziness
Less common
- Itching or rash
- Acneiform eruption
- Diarrhoea
- Flushing
Stop and get medical help if any of these occur
- Swelling of the face, lips or tongue, difficulty breathing, or collapse
- Severe muscle weakness, cramps or an irregular heartbeat early in treatment, which may indicate low potassium
- Widespread rash or hives
- Chest pain or breathlessness
- Worsening numbness or unsteadiness despite treatment

Precautions
- Establish the cause of B12 deficiency before treating. Pernicious anaemia requires lifelong replacement, and treating without diagnosis leaves the cause unaddressed.
- Correcting severe B12 deficiency can cause a rapid fall in serum potassium as red cell production resumes — monitor potassium in the first days of treatment.
- Never give folic acid alone to a patient with possible B12 deficiency. It corrects the anaemia while allowing neurological damage to progress irreversibly.
- Neurological features need prompt and adequate treatment; delay risks permanent damage.
- Multi-dose vial: use strict aseptic technique at every withdrawal, record the date of first puncture, and discard after the stated in-use period.
- Contraindicated in known hypersensitivity to cyanocobalamin or cobalt, and in Leber's hereditary optic neuropathy, where it can accelerate visual loss.
- For administration by a qualified healthcare professional, by intramuscular injection.
- Use in pregnancy and breastfeeding where clinically indicated.
